The cause of many diseases is negative emotions.

MONoneuritis (Mononeuropathy ).  


Mononeuritis is an isolated lesion of individual nerve trunks. Etiology. The basis of the disease is direct trauma, compression of the nerve trunk. Predisposing factors are the superficial location of the nerve on the bone or its passage in the tubules formed by the bone-ligamentous and muscle elements. Under such anatomical conditions, the corresponding sections of the nerve trunks are especially sensitive to chronic occupational or sports injury, compression of the limb during deep sleep, etc. (tunnel, or trap syndromes). In atherosclerosis, diabetes mellitus, periarteritis nodosa, mononeuropathies are caused by ischemia (ischemic neuropathies). Direct infection of individual nerves is very rare. In a considerable number of cases, the etiology of neuropathy is unknown. Symptoms The defeat of the main trunk of the nerve leads to the loss of all its functions. More distal foci cause only partial disturbances. The clinical picture depends on the degree of damage to nerve fibers. Only syndromes of a complete break of the main nerves of the limbs in the proximal sections, as well as the clinical course of the most common cranial mononeuropathies, are described below . 

Neuritis of the ulnar nerve.

The impossibility of bending 4 and 5 fingers and its deformation as a “clawed paw”; the last two fingers are in the state of abduction, anesthesia on the ulnar surface of the hand, as well as 4 and 5 fingers. Most often, the nerve is compressed in the cubital canal at the internal condyle of the humerus.

Neuritis of the femoral nerve.

The impossibility of extension in the knee joint, prolapse of the knee reflex, anesthesia of the anterior thigh and medial surface of the lower leg.
 

Neuritis of the radial nerve.

Impossibility of extension in the elbow of the wrist joints (“hanging wrist”), loss of reflex with m.triceps , anesthesia of the posterior surface of the shoulder, forearm and 1 finger. Most often, the nerve is damaged on the shoulder, where it spirals around the humerus.
 

Neuritis of the median nerve.

The impossibility of pronation of the hand and flexion of the first 3 fingers, anesthesia of the lateral surface of the palm, 1-3 fingers and the lateral surface of 4 fingers. Most often, the nerve is damaged in the carpal tunnel).

Neuritis of the sciatic nerve.

The impossibility of flexion in the knee joint, paralysis of the foot, prolapse of the Achilles reflex, anesthesia of the lower leg (with the exception of its medial surface) and foot.

Neuritis of the tibial nerve.

Impossibility of plantar flexion of the foot and fingers, prolapse of the Achilles reflex (walking on the fingers is impossible), anesthesia of the plantar surface of the foot. The distal nerve can be compressed in the tarsal canal at the inner ankle.

Peroneal neuritis .

Neuritis of the peroneal nerve: the impossibility of dorsiflexion of the foot (the foot hangs down and inward), anesthesia of the outer surface of the lower leg and rear of the foot.

Differential diagnosis. The differential diagnosis of mononeuropathies with radicular syndromes and plexus lesions is based on a thorough analysis of the clinical picture, which is often difficult to distinguish from radiculopathies . In favor of discogenic radiculopathies , in particular, vertebral syndrome ( analgesic scoliosis, etc. ), as well as a frequent increase in protein in cerebrospinal fluid. 

Treatment of Mononeuritis.

POLYNEURITIS ( Polyneuropathy ).  

Polyneuritis ( polyneuropathy ) is a simultaneous lesion of many peripheral nerves, manifested by symmetrical flaccid paralysis and sensitive disorders mainly in the distal parts of the extremities, with damage in some cases to the cranial nerves. 

Etiology, pathogenesis.
Intoxications (alcohol, gasoline, lead, arsenic, sulfonamides), parainfection and allergic complications ( pneumonia, mumps, etc.), dysmetabolic disorders ( diabetes, uremia, porphyria ), malnutrition (vitamin B1, B12 deficiency ), systemic diseases (periarteritis, SLE), hemoblastoses , malignant neoplasms. The severity of demyelination and degeneration of axons of nerve trunks varies depending on the etiology of polyneuropathy . The so-called infectious polyneuropathies are based on immunological disorders, and not a direct invasion of pathogens. Symptoms, course. The disease can occur at any age, but more often young people and middle-aged people. The development of painful symptoms is usually subacute or chronic (the exception is Guillain – Barré acute demyelinating polyradiculoneuropathy ). Progressive muscle weakness, atrophy, paresthesia, pain, hyperesthesia or anesthesia, decreased or loss of reflexes. These disorders are most pronounced in the distal extremities and rarely extend to the trunk. Sensitivity (vibrational more often than others) is gradually upset. Due to paralysis of the feet, steppage (cocks gait) occurs ; in severe cases, tetraplegia may occur . Muscles and nerve trunks are painful on palpation, trophic disorders occur (dryness and cyanosis of the skin, damage to the nails). Involvement in the process of vasomotor fibers of peripheral nerves causes the development of orthostatic hypotension syndrome, sometimes pupillary disorders. The nature of polyneuropathy is established only after a comprehensive examination of patients. More often than others, diabetic polyneuropathy occurs , which for many years can manifest itself only with the loss of reflexes on the legs and a violation of deep sensitivity. The development of polyneuropathy in old age requires first of all the exclusion of malignant neoplasms or myeloma . 
In differential diagnosis polymyositis (there are no disturbances in sensitivity and loss of reflexes), poliomyelitis (purely motor impairments) and dorsal dryness (lack of atrophy, the presence of pupillary anomalies) should be kept in mind . It is also necessary to take into account the subclinical course of polyneuropathy , when the disease for a long time manifests itself as a lesion of only one or two nerves, and only a thorough clinical examination and determination of the speed of conduction along the nerves of the limbs allow us to establish the polyneuropathic nature of the process. In many cases of polyneuropathy, the level of protein in the cerebrospinal fluid is moderately elevated, which indicates the involvement of not only peripheral nerves, but also roots ( polyradiculoneuropathy ). Treatment of Polyneuritis. 
With alcoholic polyneuropathy, parenteral administration of thiamine (vitamin B1) up to 1000 mg / day (20 ampoules of 5% solution),  In case of metal poisoning – chelates EDTA , etc.),In porphyria , adenyl .
